Knowing that death is near, it is important for your loved one to communicate with you (while they still have the capacity to communicate) to let you know how they would like to be treated in their final days. The Five Wishes booklet is a useful tool to help your loved on talk about how they would like to be treated. Tom Torkildson, from our staff, would be happy to help you fill out the Five Wishes booklet. Call Tom at 970-353-1212 to set an appointment.
